Firefighter Registration Instructions

Firefighters employed on or after July 1, 2001

  • Firefighters employed on or after July 1, 2001 must complete Section A and Section B of the Firefighter Registration form.  Form must be signed by the Fire Chief or his/her designee.
  • Request for named based criminal records check must be submitted to the Office of State Fire Marshal, who will in turn inform the requesting department of the results of the background check through SLED.
  • Completion of Section A and Section B required for all actions taken after initial registration.
  • All information on form must be completed.
  • All forms must be typed or printed legibly.
